1. Informed Consent Requirement
- All clients must provide informed consent before any treatment or service is performed. This includes sports massage therapy, personal training, and any other services offered by Zenith Performance and Recovery.
2. Process for Obtaining Informed Consent
- Initial Consultation: During the initial consultation, the practitioner will explain the details of the proposed treatment, including:
- The purpose of the treatment.
- The specific techniques to be used.
- Any potential risks or discomfort that may arise.
- Expected outcomes or benefits of the treatment.
- Opportunity for Questions: Clients will have the opportunity to ask questions about the treatment, the process, and any potential risks. The practitioner will ensure that all questions are answered clearly and comprehensively.
3. Written Consent
- Consent Form Delivery: The consent form will be sent to the client via email when the appointment is confirmed. This allows the client time to review the form and raise any questions before the appointment.
- Signing the Consent Form: While the consent form does not need to be signed before the appointment to allow for discussion, it must be signed before the treatment begins. This ensures that the client fully understands the treatment and has provided formal consent to proceed.
4. Special Consent Considerations
- For Clients Under 18: Informed consent must be obtained from both the client and their parent or legal guardian before any treatment is performed.
- Ongoing Treatments: For clients receiving multiple or ongoing treatments, consent will be re-confirmed verbally at each session, with the option to review or amend the written consent if necessary.
5. Right to Withdraw Consent
- Clients have the right to withdraw their consent at any time, even during a treatment session. If a client expresses discomfort or wishes to stop the treatment, the practitioner will immediately cease the session and discuss alternative options.
6. Record Keeping
- Signed consent forms will be stored securely in the client’s file. These records will be retained for as long as legally required and will be accessible only to authorised personnel.
7. Adjustments to Treatment
- If any significant changes to the planned treatment arise (e.g., new techniques or procedures), the practitioner will explain these adjustments to the client and obtain fresh consent before proceeding.
8. Confidentiality
- All information discussed during the informed consent process, including health-related disclosures, will be kept confidential in accordance with the Privacy Policy and Data Protection Policy of Zenith Performance and Recovery.
